The Dark Backward

The Dark Backward is a 1991 American black comedy film written and directed by Adam Rifkin. It stars Judd Nelson, Bill Paxton, and Wayne Newton. It follows a garbage man who tries his hand at stand-up comedy, failing miserably until a third arm mysteriously grows from his back.
